Why Apollo Root Cause Analysis
Apollo challenges the assumption that causation is linear. Most failures have multiple interacting causes. Finding the single “root cause” is a simplification that leads to repeat failures. See the Apollo RCA method explained in full on our methodology page.
Built for Queensland’s Heavy Industry
Our Brisbane-based training draws on real failure scenarios from the industries that drive Queensland’s economy. Participants work through examples relevant to their operations.
Coal Mining
Dragline structural failures, longwall shearer breakdowns, conveyor belt fires, truck suspension fatigue. Bowen Basin operators including Glencore, Anglo American, BHP Mitsubishi Alliance, Whitehaven Coal, and New Hope Group use structured RCA to reduce unplanned downtime across their fleets.
Energy and Power Generation
Turbine blade degradation, boiler tube failures, transformer insulation breakdowns, cooling water system corrosion. SEQ generators such as Stanwell and CS Energy rely on systematic cause-and-effect analysis to maintain grid reliability and extend asset life.
Minerals Processing
Crusher liner wear, cyclone overflow issues, thickener rake failures, SAG mill bearing degradation. Operations across Gladstone and the broader Queensland minerals corridor benefit from Apollo RCA to address chronic equipment losses that erode throughput.

Designed for Operational Teams
Apollo RCA training is built for the people who investigate failures, make maintenance decisions, and manage asset risk in heavy industry.
Reliability Engineers
Lead structured investigations, build RealityCharts from equipment failure data, and identify the causes that linear methods miss. The core audience for Apollo certification.
Maintenance Managers and Planners
Connect investigation findings to maintenance strategy. Use RCA outputs to justify PM interval changes, parts standardisation, or design modifications backed by evidence.
Operations and Production Leaders
Participate in cross-functional investigations as operational subject matter experts. Understand how operational decisions create conditions for equipment failure.
HSE and Risk Professionals
Apply Apollo to safety incidents and near-miss investigations. Build cause-and-effect diagrams that satisfy regulatory reporting while identifying systemic hazards.

Frequently Asked Questions
Where in Brisbane is the training held?
Public courses run at venues in the Brisbane CBD or inner suburbs. On-site training is delivered at your facility anywhere in the Brisbane metro area, Sunshine Coast, Gold Coast, or wider South East Queensland. HolisticAM is headquartered in Maroochydore with a Brisbane office, so there are no travel surcharges for SEQ delivery.
Can you deliver on-site training at our Bowen Basin mine site?
Yes. We regularly deliver Apollo RCA training at mine sites across the Bowen Basin, including operations run by Glencore, Anglo American, BHP Mitsubishi Alliance, Whitehaven Coal, and New Hope Group. On-site training uses your own equipment failure examples and can be tailored to your site procedures.
How is Apollo RCA different from 5-Whys?
5-Whys is a tool, not a method. It follows a single causal chain and stops when someone decides the answer sounds reasonable. Apollo uses cause-and-effect principles to map every causal path through a RealityChart, exposing multiple interacting causes that 5-Whys misses entirely. The result is effective solutions that actually prevent recurrence, rather than recommendations that address only one contributing factor.
Do participants need prior RCA experience?
No. The course starts from first principles. It is designed for reliability engineers, maintenance managers, HSE professionals, operations leaders, and asset managers at any experience level. Participants who have used other RCA methods will recognise where Apollo addresses the limitations they have likely encountered.
What software is included with the training?
Every participant receives a 12-month licence for RC Pro, the investigation management software developed by Apollonian Publications. RC Pro is used throughout the course to build RealityCharts, document evidence, assign corrective actions, and generate reports. Your licence and support come direct from HolisticAM, with local Australian support.
Who is the Apollo RCA instructor?
All HolisticAM Apollo RCA training is delivered by Nik Sharpe, our certified Apollo RCA instructor. Nik is a Senior Reliability Engineer with extensive field experience across mining, energy, and heavy industry. HolisticAM is an Apollo-accredited training organisation, certified by Apollonian Publications and backed by Australia’s largest reliability engineering team.
