Why Apollo Root Cause Analysis

Apollo challenges the notion that causation is linear. Instead of chasing a single root cause down one causal chain, Apollo maps all causes and their relationships to find effective solutions that prevent recurrence. Read how the Apollo RCA methodology works for the full explanation.

Built for WA’s Toughest Industries

Our training uses failure scenarios drawn from the industries that drive Western Australia’s economy. Participants work through problems they recognise from their own operations.

Iron Ore

Pilbara operations face extreme heat, dust ingress, and conveyor system failures that can bring an entire export chain to a halt. Apollo RCA maps the causal chains behind crusher bearing failures, rail car derailments, and screening plant breakdowns that reduce throughput across BHP, Rio Tinto, and FMG operations.

Gold Mining

Processing plant failures in the Goldfields directly impact recovery rates and production targets. Apollo RCA addresses recurring mill liner failures, leach circuit upsets, and flotation cell performance degradation across operations like Northern Star’s Kalgoorlie assets and Gold Fields’ St Ives and Granny Smith mines.

LNG and Oil and Gas

NW Shelf and Browse Basin LNG facilities operated by Woodside and Chevron face compressor trips, heat exchanger fouling, and instrument failures that trigger costly production curtailments. Apollo RCA’s structured approach meets the rigorous investigation standards required in Process Safety Management frameworks.

Alumina and Heavy Industry

Kwinana industrial strip and Pinjarra refinery operations managed by Alcoa and South32 deal with digester scaling, calciner refractory failures, and pump seal degradation. Apollo RCA helps maintenance teams move beyond repeat repairs to solutions that address the underlying causal structure of chronic equipment problems.

Built for Technical Leaders

Apollo RCA training equips the people closest to your assets with a structured method for preventing failure recurrence.

Maintenance Managers

Lead investigations that go beyond component replacement. Build a culture where maintenance teams identify and address systemic causes rather than accepting repeat failures as normal.

Reliability Engineers

Add a structured investigation methodology to your reliability toolkit. Apollo RCA complements FMECA, RCM, and condition monitoring by addressing failures that have already occurred and preventing their recurrence.

Operations Leaders

Understand the causal structure behind production losses and process upsets. Participate effectively in cross-functional RCA investigations and hold teams accountable for solution implementation.

Safety and Risk Professionals

Meet investigation requirements under WA mining regulations and Process Safety Management standards. Apollo RCA provides the rigour and documentation needed for regulatory submissions and incident reporting.

Everything Included in Your Training

No hidden costs. Every participant receives the full certification package.

Apollo RCA Facilitator Certification

Accredited by Apollonian Publications. Recognised internationally.

12-Month RC Pro Software Licence

The industry-standard tool for building, storing, and sharing RealityCharting cause-and-effect diagrams.

Frequently Asked Questions

Do you run Apollo RCA public courses in Perth?
Yes. We run scheduled public Apollo RCA courses in Perth throughout the year. Check our training calendar for upcoming dates. We also deliver on-site training at your Perth, Pilbara, or Goldfields facility if you have a group of 6 or more participants.
Can you deliver training at our Pilbara site?
Yes. HolisticAM has a Perth office and regularly delivers on-site training across WA, including Pilbara iron ore operations, Goldfields mine sites, and Kwinana industrial facilities. On-site training uses your own failure examples and can be scheduled around shutdown windows and roster cycles.
How is Apollo RCA different from TapRooT, ICAM, or 5-Whys?
Apollo uses cause-and-effect principles to map all causal paths, not just the most obvious one. Unlike 5-Whys (which follows a single chain), Apollo creates a RealityCharting diagram that exposes conditional and action causes simultaneously. Unlike category-based methods, Apollo does not force causes into predefined boxes. The focus is on finding effective solutions, not assigning blame or fitting a template.
Who delivers the training?
Nik Sharpe, our certified Apollo RCA instructor, delivers all training. Nik is a Senior Reliability Engineer with extensive field experience across mining and heavy industry. HolisticAM is an Apollo-accredited training organisation, certified by Apollonian Publications and backed by Australia’s largest reliability engineering team.
What software is included and how long does the licence last?
Every participant receives a 12-month licence for RC Pro, the industry-standard software for building RealityCharting cause-and-effect diagrams. Licence renewals are available directly through HolisticAM after the initial 12 months, with local Australian support.
Is the certification recognised by WA mining regulators?
Apollo RCA certification is recognised internationally and is used by major mining, oil and gas, and utilities organisations in WA. While no specific RCA methodology is mandated by DMIRS, the structured documentation produced by Apollo RCA meets the investigation and reporting standards expected under WA mining safety legislation.
