What a Manufacturing RCA Investigation Looks Like
Apollo RCA follows a structured four-step method. Applied to a manufacturing failure, this is what the process looks like in practice.
Step 1: Define the problem statement. The investigation begins by writing a precise problem statement — what failed, when it occurred, and what the impact was in production terms. “The filling line tripped at 14:32 on Tuesday, resulting in 4.2 hours of unplanned downtime and 800kg of product loss.” This bounds the investigation and stops it expanding into every adjacent issue the team has been frustrated about for months.
Step 2: Map the causal chain. The team builds a RealityChart — a visual logic tree that traces every causal path from the immediate physical cause back through conditional causes to the organisational factors that allowed them. A filling line stoppage might trace from a worn seal (physical) through to a PM interval that had been extended under production pressure (conditional) and an absence of any condition monitoring on that component (organisational gap). All three levels need corrective actions. Addressing only the physical cause means the failure will return under the same organisational conditions.
Step 3: Identify corrective actions. Apollo does not produce a single corrective action — it produces actions at multiple points in the causal chain. Each action is evaluated for feasibility, whether it can prevent recurrence, and whether it introduces new risks. This is where Apollo diverges from most RCA methods. The question is not “what did we do wrong?” but “which interventions, at which points in the causal chain, give the highest probability of preventing this happening again?”
Step 4: Assign, track, and verify. Corrective actions are assigned with owners, deadlines, and success criteria and tracked in RC Pro software. The investigation is not complete when the report is written — it is complete when the actions are implemented and the team has verified that the failure rate has changed.
